Incoming Student-Athlete Checklist

All Student-Athletes

Eligibility & PlayNAIA 

  • Create PlayNAIA account and submit all eligibility documents
    • Upload all official high school and any college credit transcripts
    • Send official ACT/SAT scores to NAIA Eligibility Center (if needed to meet eligibility standards) 
    • Pay NAIA Eligibility Center fee (or document fee waiver, if applicable) 
  • Send ECID# to head coach to be added to the team's short list
  • Approve coach’s short list request in PlayNAIA/ECP once invited

Admissions

  • Submit valid photo ID to admissions at admissions@ussu.edu
  • Confirm all official final transcripts are received by the institution (high school, dual enrollment, and all prior colleges/universities) 
  • Sign enrollment agreement (sent to @students.ussu.edu email upon acceptance to university)

Financial Aid 

  • Submit FAFSA application
  • Register for classes with academic advisor (must be full-time for eligibility) 
  • Sign scholarship agreement (sent to @students.ussu.edu email, if applicable) 
  • Set up student billing payment plan (if desired)

Athletic Training

  • Submit completed physical exam and paperwork to athletictraining@ussu.edu as well bring in person to Head Athletic Trainer
  • Complete registration for SportsWare, USSU's athletic training software
    • Use School ID code: USSUEagles
  • Show proof of primary health insurance card OR sign secondary insurance waiver (per institutional policy) 
    • If you don't currently have primary health insurance but would like to purchase athletic coverage through the school, sign up here.

Other

Transfer Students-Athletes ONLY

* These items are in addition to the above list for any student with prior collegiate identification.

Transfer Eligibility

  • Request tracers from all previous colleges (including any non-NAIA schools), with accurate terms of attendance and seasons of competition listed 
  • Sign the Transfer Information form (sent to @students.ussu.edu email)
  • Meet with Faculty Athletics Representative (FAR) to review eligibility, residency, and progress-toward-degree status
  • With FAR, complete NAIA Transfer Player Eligibility Statement (via ECP) before practice or competition 
  • Complete the outside competition form, if there was any non-collegiate competition that could affect seasons of competition (especially track & field, wrestling, soccer, etc.)
  • All official college transcripts received and evaluated by the registrar for degree applicability (needed for 24/36-Hour and Progress Rules) 

International Students ONLY

* These items are in addition to the all student-athlete list for any student not classified as a U.S. Citizen
  • Obtain primary health insurance
