Incoming Student-Athlete Checklist

ALL STUDENT-ATHLETES

1. Eligibility & Compliance

  • Create PlayNAIA account and complete the NAIA Eligbility Center registration process.
    • Submit all academic records requested by the NAIA Eligbility Center, including official high school and/or college transcripts as applicable.
    • Submit official ACT/SAT scores if required for your eligibility determination.
    • Pay the NAIA Eligibility Center fee or submit documentation for an approved fee waiver, if applicable. 
  • Send your NAIA ECID# to head coach to be added to the team's short list.
  • Approve coach's short list request in PlayNAIA/ECP once invited.
  • Receive required NAIA and institutional eligibility clearance before participating in outside competition.

2. Admissions

  • Submit valid photo ID to admissions at admissions@ussu.edu.
  • Confirm USSU has received all required official final transcripts, including:
    • High school transcript
    • Dual-enrollment transcripts
    • Transcripts from every previously attended college/ university
  • Sign the USSU Enrollment Agreement sent to your @students.ussu.edu email.
  • Submit a personal photo for your Sammy ID card to admissions@ussu.edu.

3. Financial Aid & Student Account

  • Submit the FAFSA application, if seeking federal financial aid.
  • Register for classes with your academic advisor and maintain the credit-hour enrollment required for NAIA eligibility.
  • Sign your athletic/institutional scholarship agreement, if application. Agreements are sent to your @students.ussu.edu email.
  • Set up a student billing payment plan, if needed.
  • Confirm that your financial aid and student account requirements are complete.

4. Sports Medicine

  • Complete the required pre-participation physical exam and athletic training paperwork.
  • Submit required medical paperwork to athletictraining@ussu.edu and provide any documents requested by the Head Athletic Trainer.
  • Complete SportsWare registration.
    • School ID: USSUEagles
  • Complete the appropriate health insurance requirement:
    • If you have qualifying primary health insurance, submit a current copy (front and back) of your health insurance card to USSU Sports Medicine.
    • If you do not have qualifying primary health insurance, purchase the USSU student-athlete insurance plan.
    • If your health coverage is through a payer-of-last-resort or similar program (such as Medicaid, certain military/governmental programs, tribal health programs, or other coverage that may not function as primary insurance for athletic injuries), purchase the USSU student-athlete insurance plan OR sign the USSU Secondary Insurance Waiver, if applicable.
  • Receive medical clearance from Head Athletic Trainer before participation.

5. Student-Athlete Information

 

TRANSFER STUDENT-ATHLETES

Complete these requirements in addition to the All Student-Athletes section if you have previously attended a college or university.

1. Transfer Eligibility & Compliance

  • Disclose every college/university previously attended, including institutions attended for dual enrollment, part-time enrollment, or other collegiate coursework.
  • Ensure USSU has received an official transcript from every previously attended college/university.
    • Registrar and/or Compliance Officer with review for degree applicability (needed for 24/36-Hour and Progress Rules).
  • USSU will request a transfer-tracer from all previously attended institutions.
  • Complete and sign all transfer eligibility documentation with the Faculty Athletics Representative (FAR), including NAIA Transfer Player Eligibility Statment and Competitive Experience Statement.
    • Fully disclose all previous collegiate athletic participation and seasons of competition.
    • Report any outside or non-collegiate competitive participation that occurred after high school.
  • Complete any additional eligibility review requested by the FAR, Registrar, or Compliance Officer.
 

INTERNATIONAL STUDENT-ATHLETES

Complete these requirements in addition to the All Student-Athletes section if you have international academic records or otherwise require international eligibility processing.

1. International Eligibility & Compliance

  • Register with the NAIA EligibilityCenter through PlayNAIA. 
  • Register with InCred and purchase the required international credential evaluation.
  • Submit all required secondary-school and postsecondary academic records to InCred.
    • If academic records are not issued in English, submit the academic record in their original language, and a literal English translation
  • Complete any additional documentation requested by the NAIA Eligibility Center or InCred.
  • Receive the required NAIA Eligibility Center determination before competition.

2. Admissions & Enrollment

  • Complete all USSU international admissions and enrollment requirements.
  • Submit all documents required for your student immigration status and USSU international student file.
  • Maintain required immigration/student status and enrollment requirements.

3. Sports Medicine

  • Provide proof of current health insurance to USSU Sports Medicine.
  • Receive medical clearance from USSU Head Athletic Trainer before participation.
Notify the appropriate USSU office immediately of any change that could affect your immigration, enrollment, or eligibility status.
