MEDICAL FORMS
Before participating in any intercollegiate athletic activities, all USSU student-athletes must complete the required medical forms and satisfy the applicable health insurance requirements. All documentation must be received and approved by USSU Sports Medicine before the student-athlete is medically cleared for participation in practices, competitions, or team-related training sessions.

PRIMARY INSURANCE
Participating in intercollegiate athletics carries inherent risk of injury. Health insurance is an important part of ensuring student-athlete have access to appropriat emedical care and protection from potentially significant medical expenses.
All USSU student-athletes are required to obtain and maintain qualifying primary health insurance as a condition of participation in intercollegiate athletics. Student-athletes must provide USSU Sports Medicine with a current copy of the front and back of their health insurance card before receiving medical clearance.
Student-Athletes Without Qualifying Primary Insurance
Student-athletes who do not currently have qualifying primary health insurance must obtain coverage before being medically cleared for athletic participation.
This includes student-athletes whose existing coverage is through a payer-of-last-resort or similar program, such as Medicaid, certain military/governmental programs, tribal health programs, or other coverage that may not function as primary insurance for athletic injuries.
Student-athletes without qualifying primary health insurance must:
- Obtain qualifying primary health insurance by either:
- Sign the USSU Secondary Insurance Waiver and Acknowledgment if qualifying primary insurance has not yet been obtained. The waiver documents the student-athlete's current insurance status but does not satisfy USSU's primary health insurance requirement and does not provide access to the University's secondary athletic insurance.
Student-athletes without qualifying primary health insurance will
not be medically cleared for athletic participation until qualifying coverage has been obtained and verified by USSU Sports Medicine.

INTERNATIONAL PRIMARY INSURANCE
International student-athletes are required to obtain and maintain primary health insurance that meets USSU requirements.
International student-athletes should review the International Student Protection Packet and complete the International Student Protection Online Enrollment, as applicable.
Proof of active coverage must be provided to USSU Sports Medicine.
SECONDARY INSURANCEÂ
USSU maintains an athletic accident insurance policy that serves as secondary coverage for eligible student-athletes.
The University's secondary athletic insurance is available only after the student-athlete's qualifying primary insurance has been billed and processed. Coverage is subject to the terms, conditions, limitations, exclusions, deductibles, and benefit provisions of the University's policy.
Student-athletes without qualifying primary health insurance are not eligible to access USSU's secondary athletic insurance coverage and may be personally responsible for medical expenses associated with athletic injuries.
Student-athletes do not separately enroll in USSU's secondary athletic insurance. As long as they are enrolled as active full-time student-athletes at USSU, they are automatically included under the policy. Questions regarding coverage or the claims process should be directed to USSU Sports Medicine.
BANNED SUBSTANCES & DRUG TEST POLICY
USSU is committed to maintaining a safe, healthy, and drug-free environment for all student-athletes. Student-athletes are subject to the University's drug testing policies and procedures as outlined in the USSU Student-Athlete Handbook.
All student-athletes are required to complete the applicable Drug Testing Consent Form as a condition of participation.
Student-athletes are responsible for reviewing and complying with the University's policies regarding banned substances, drug testing, prescription medications, and supplements.
