Medical Forms

Prior to participating in any athletic activities, all student-athletes at the United States Sports University are required to complete the necessary medical forms and submit proof of active primary health insurance. These documents must be received and approved by Sports Medicine before the student-athlete is cleared for practices, competitions, or team-related training sessions.

Primary Insurance

Participating in sports carries inherent risks, and the possibility of a serious injury is always present. Having proper health insurance ensures you receive the necessary medical care without facing overwhelming financial burdens. Adequate coverage protects you in the event of an accident or injury, which can occur at any time during training or competition. At the United States Sports University (USSU), all student-athletes are required to have active primary health insurance coverage before participating in intercollegiate athletics.

For student-athletes who lack primary insurance or have limited coverage (such as Medicaid or Military insurance), USSU requires the purchase of the 1st Care Plan through First Agency. This policy provides basic athletic accident coverage and is valid for one full year. It covers 100% of usual, reasonable, and customary charges related to athletic injuries. Please note that USSU will be the official policyholder—not the individual student-athlete—and the University will handle all claim submissions on your behalf.

Secondary Insurance

The United States Sports University maintains a base athletic accident insurance policy that serves as secondary coverage for its student-athletes. This policy is used only after the student-athlete’s primary insurance has been applied and its benefits have been exhausted. Coverage applies exclusively to injuries that occur during participation in officially sanctioned athletic activities, including practices, competitions, and team-organized strength and conditioning sessions.

Student-athletes do not need to purchase or register for this coverage. As long as they are enrolled as active full-time student-athletes at USSU, they are automatically included under the policy.

Drug Testing Policy

The United States Sports University is committed to maintaining a safe, healthy, and drug-free environment for all student-athletes. As part of this commitment, USSU conducts both random and for-cause drug testing throughout the academic year.

All student-athletes are required to sign a consent and release form each year as a condition of participation. Failure to comply with the policy or testing procedures may result in suspension, loss of eligibility, or termination of athletic aid.

Banned substances include illegal drugs, controlled substances, and prescription medications not authorized by a physician and cleared by the Head Athletic Trainer. A list of banned substances in provided to student athletes each year. USSU maintains strict confidentiality and uses certified laboratories to ensure accurate and secure testing.
